Southwest Airlines investigation

The fatal Southwest Airlines accident may have all been triggered by a broken fan blade. The NTSB confirmed that one of the 24 fan blades inside the Boeing 737's engine snapped mid-flight. The engine debris smashed a window, and a passenger was partially sucked out. A medical examiner said she died of blunt-force trauma.
